MyMedicare
MyMedicare is a voluntary and free Australian Government program that allows you to register your regular general practice and preferred GP. It is designed to strengthen the relationship between you, your GP and your healthcare team and support greater continuity of care.
Why register with Elder Street Practice?
Registering Elder Street Practice as your preferred practice may give you access to additional Medicare-supported services, including:
- Longer Medicare-funded telehealth consultations with your registered practice and GP, where clinically appropriate.
- Greater continuity of care with your regular GP and practice team.
- Access to MyMedicare-linked programs and services where eligible, including additional support for people living in residential aged care.
- MyMedicare is also increasingly being used to support continuity of care for patients with chronic and complex health conditions.
Registration is voluntary and free, and you can change your preferred GP or practice, or withdraw your registration, at any time. You can still see other GPs, including other doctors within Elder Street Practice.
How to register
You can register in one of the following ways:
1. Register at the practice
Ask our reception team for a MyMedicare Registration Form. Complete and sign the form and we can process your registration.
2. Complete a registration form
Download the current Australian Government MyMedicare Registration Form, complete and sign it, and return it to Elder Street Practice.

3. Register online through myGov
You can register through your Medicare online account:
- Sign in to myGov and select Medicare.
- Select View MyMedicare, then Start.
- Search for and select Elder Street Practice.
- Select your preferred GP.
- Complete the optional information if you wish.
- Select Submit registration and follow the prompts.

You can also manage Medicare services through the Express Plus Medicare mobile app.
Who can register?
Patients with a Medicare card or Department of Veterans' Affairs (DVA) Veteran Card may be eligible to register. Eligibility requirements apply, including requirements relating to your previous visits to the practice in some circumstances.
